What is Linkage?

Number One

Meet & Greet

You receive initial intake into one of our partnering clinics. Your labs are drawn and you receive consultation from their Nursing Case Manager. You are prescribed a recommended therapy to manage your HIV after receiving results from our labs detailing.

Number 2

The Numbers

Current viral load - the concentration of virus within your system
Current T-cell count - the number of white blood cells still actively protecting your body.

Number 3

Viral Supression

TruEvolution follows you to ensure you have achieved viral suppression, or undetectable status, upon the 2nd follow-up appointment (3rd visit to the clinic). This means the HIV virus can no longer be detected with HIV testing, and this undetectable status can remain with ongoing treatment. This also means the virus cannot be passed on to other people, what is known as U=U (undetectable = untransmittable).

Number 4

Quality of Life

Upon preliminary diagnosis, TruEvolution immediately offers you referral into the organization’s clinical therapy program where you will receive both one-to-one mental health assessment as well as referral into our weekly support groups overseen by a clinical therapist. If you indicate you need ongoing psychotherapeutic and counseling services, then you will be enrolled into our behavioral health program.

Case Management

As part of the organization’s commitment to enhancing the quality of life for the clients we serve, TruEvolution will guide and support clients so that they will:

  • Understand the healthcare system
  • Navigate benefits counseling and insurance enrollment
  • Access linkage to primary care services
  • Have access to Spanish translation services
  • Obtain referrals to any specialty or supportive services needed

For Inland Empire Health Plan (IEHP)-eligible clients, TruEvolution Care Coordinators are able to facilitate medical transportation and linkage to on-site mental health and emotional support services. If a person who has fallen out of care or is looking to engage in HIV care, prevention, or other primary care services, they may contact care@truevolution.org or call 951-888-1346, extension 1.
